TRANSURETHRAL PHARMACOTHERAPY: MUSE

Alternative methods of delivering medication into the shaft of the penis have always been of major research interest. Such methods offer the patient a less invasive treatment option. Research in this area has initially shown that the absorption of these drugs through the skin was too difficult. This research effort was then directed towards finding a treatment that could pass through the urethra (the tube that runs along the penis which enables you tourinate standing up).
In 1997, a drug formulation was developed and approved for use in the United States that did just that. This medication is called Muse and has not yet been released in Australia. With this proceedure a medicated pellet is inserted through a special delivery system that places the pellet deep down the urethra. Muse is a pellet form of Prostaglandin Ei. Up to 65 per cent of men using this treatment achieve an erection sufficient for intercourse. Side effects of this drug include:
•          pain in 30 per cent of patients who use the the treatment
•          bleeding from inside the urethra in 5 per cent of patients
•          dizziness in 2 per cent of patients.

Priapism was shown to be a very rare occurrence.
The efficacy and safety profile of Muse make this new formulation a suitable alternative for patients with psychogenic symptoms. This form of therapy may also be a useful treatment for those men who have a mild vascular problem which is causing their erectile failure. Unfortunately, in the United States, the release of Viagra has led to a significant decline in the use of this type of treatment. However, there are still many instances where this treatment is used.
