The hospital service has also undergone fundamental changes under the government's NHS reforms. One part of the reform is an identification of who gives the care (providers) and who buys it (purchasers). In most areas a health authority and GP (general practitioner) fund-holders buy care in the form of contracts. The contract is the agreement between the purchaser and the provider (hospital) for a patient to be seen, or an operation to be performed for a certain agreed price. Many contracts are block contracts, i.e. it includes all work done for a whole group of patients (e.g. all new hips, all heart bypass operations or all elderly care etc.) Specialist work can still be organized but on an individual contract level. All emergency work, however, does not depend on contracts in the same sense (the charge is made to the relevant authority after the event) and hence should be and is immediate via the Accident and Emergency department. The main providers of this form of health care are hospitals. The reforms affected hospitals, which were granted Trust status - initially only a few, but now the majority in the UK. In a similar way to fund-holding GPs, hospitals have been encouraged to become Trusts once they have demonstrated to the government financial security. They are then given a greater degree of flexibility in how they function (types of service, pay rates, etc.) within certain constraints. The idea is that the providers (hospitals) compete for contracts from purchasers (health authority and GP fund-holders) by offering as low a price as possible per contract, combined with examples of good practice (better discharge planning, easy access to specialists by phone, shorter waiting times etc.) To implement these changes the government has had to spend millions of pounds on administrators (the 'in' term is resource management). To compete, win contracts and avoid bankruptcy (essential for a hospital's survival now) the doctors, nurses and managers have had to have business training and view their service from that perspective. Some aspects of these reforms are obviously good; wasting resources has been virtually abolished. Checking on how we function and improving the system if flaws are found (audit) has been professionally constructive and beneficial to patients. One huge problem remains however, the NHS is still vastly under-funded. Thus everyone is trying to buy more for less and do more for less, a recipe for business disaster. Demand continues to increase and medical and social advances continue to outstrip the budget.
The National Health Service is such a broad-based organization that it copes with the acute accident victim at one end of a spectrum, to the chronically handicapped frail immobile person at the other. All areas have an accident and emergency department open 24 hours, basically capable of coping with anyone sent to it. Unless it is an extreme emergency however the person should have contacted their GP first. In some areas the A&E department serves as a GP clinic and this is wrong. It is wasteful of time and resources and this situation will hopefully improve as the standard of general practice improves and the awareness and education of the general public gets better.
