
BABY AND CHILDHOOD DISORDERS OF THE EARS, NOSE AND THROAT: NECK SWELLINGS (CERVICAL ADENITIS)
A large single swelling may occur in the neck under the jaw, and it may come on suddenly and be extremely tender. The child may feel off-colour, hot and feverish and tender all over. Often this follows or accompanies a sore throat, acute tonsillitis, sinus or ear infection, or upper respiratory tract infection. It usually means the germs have gained access to the lymph gland in the neck and have involved it in the infection. The swelling may be extremely tender, and the skin overlying it may become red, inflamed and hot. The infection may occur on one or both sides.
Treatment
This may represent a gland infection secondary to an ear-nose-throat infection or upper respiratory tract infection. Or it may be unrelated, and indicate glandular fever or some more serious disorder.
Swellings that do not settle in a short time with simple therapy must be examined by the doctor for proper diagnosis and treatment. Treatment will depend on the diagnosis: antibiotics will probably be given for proven cases where this may cure the disorder, but antibiotics are unlikely to help viral-caused glandular infections. Antipyretics and analgesics, plenty of fluids, soft foods and bed rest are all part of the treatment.
