Spina bifida, or myelodysplasia if you prefer long and unpronounceable words, is a developmental abnormality of the brain system. Fortunately it is rare, and most doctors in general family practice may never see one in a professional lifetime. It really means there has been a defect in the development of the spinal cord, usually caused by the canal not forming correctly and not sealing over. The lower part of the back is the commonest site.
So-called spina bifida occulta is the most frequent, but even this is uncommon. There may be no symptoms at all. There may be a gradual weakening of control of the bladder or bowels, making toilet training more and more difficult. There may be weakness of the legs and paralysis may gradually set it.
However, anomalies in other parts of the cord may occur, and a lump visible externally may grow. The nature and extent of symptoms various enormously according to the extent of the defect.
Treatment
Babies and children with this condition are often treated surgically, but each case is individually assessed, usually in the neurological department of a major public hospital which is geared to cope with conditions like this. Some cases gain good results. I recall a patient of many years' standing who had mild spina bifida, and he lived a relatively normal life well into his forties. But many others are not so fortunate.
Obvious cases at birth will automatically be channelled into the specialized units that are available in hospitals in most large cities.
There, intensely specialized care will be given, and attempts at surgical correction may be made.
Others whose symptoms are minimal may not be diagnosed until later in life. Then, they will also be cared for in the specialized neurology department of the larger hospitals by specialist surgeons called neurosurgeons.
